The contractor shall provide all necessary personnel, transportation and malpractice insurance to provide urology service to Indian patients at the Gallup Indian Medical Center. The Contractor shall be board certified in urology and licensed in the State of New Mexico. The estimated workload is two (2) to three (3) major cases per visit (prostatic and renal surgery), and an estimated four (4) minor cases per visit (cystoscopies, TURP, etc.). The annual visits are 50 visits. Contract to be effective for a twelve (12) month period with two (2) one-year option periods.
