Tokai-Hokuriku Regional medical Affairs Bureau and comprised of firms
that meet the following criteria 1) firms that do not fall under
articles 7O and 71 of the cabinet order concerning Budget, auditing,
and Accounting; 2) firms pre-qualified for open competitive tendering
on Construction works by the Tokai-Hokuriku medical Affairs Bureau; 3)
firms that score 1,100 points or more in a formula calculated from
bidding company's objective criteria at the time of application for
qualification; 4) firms not undergoing bankruptcy procedures; 5) firms
that have since January 1990 actually carried out Construction of the
following (B) -- in the case of consortium, (a) a) hospital or Office
building made of reinforced concrete or steel-framed reinforced
concrete, at least one story below ground and five stories above
ground, of at least 18,000 square meters land area; and (B) hospital or
Office building made of reinforced concrete or steel- framed reinforced
concrete, at lease one story below ground and five stories above
ground; 6) firms not currently suspended from designation by the
Director of the Tokai-Hokuriku medical Affairs Bureau; 7) firms not
related by capital or personnel to the contractors involved in this
project; and 8) designated consortia must be able to assign to the
Construction site a full- time chief engineer or engineering supervisor
who meets the following criteria a) is qualified as a certified
first-class Construction supervisor or architect; B) has since January
1990 had Construction experience as engineering supervisor, is
qualified to supervise designated Construction works bid documents must
be obtained directly from the submitting agency, not from U S. Embassy.
