R -- ANNUAL MARKET SURVEY FOR COMPREHENSIVE WELLNESS/FITNESS PROGRAM FOR THE OMAHA DISTRICT CORPS OF ENGINEERS POC For further information, Contact Lynda VanHouse at: (402) 221-4076. The Wellness/Fitness Program will be provided at 215 North 17th Street, Omaha, NE and numerous field site locations. The intent of the Government is to negotiate an interagency agreement with the U.S. Publich Health Service Region VII, unless this notice reveals additional contractors or vendors capable of satisfying the requirements. The primary objectives of the Wellness/Fitness Program is to: (1) provide a comprehensive program designed to improve the health, vitality, and performance of Omaha District employees; (2) promote health and wellness to all employees as a means of reducing risks associated with heart disease and other preventable chronic illnesses; (3) promote health and wellness programs to assist managers in reducing expenditures related to health care of employees and in reducing absenteeism. The Contractor is to provide two wellness/fitness health professionals (4000 hours) who will provide a minimum of 150 health risk assessments (HRA) and fitness assessments and evaluations. The assessments include an analysis of the employees lifestyle and medical history, measurement of height, weight, blood pressure, a blood test to measure glucose, cholesterol and triglycerides (total HDL & LDL), measurement of flexibility and strength, endurance, body composition, submaximal exercise test, and employee counseling on the results of the assessments. The contractor will provide health education to encourage employees to maintain personal health; and establish and operate physical fitness programs and facilities deisgned to promote and maintain employee health and physical fitness for approximately 3000 employees. Occasional travel to Colorado, Montana, North & South Dakota is required. The Government will use the information obtained from this notice to evaluate the capabilities of the marketplace. Interested vendors/contractors must submit clear and convincing documentation which clearly demonstrates their capability to meet the Governments requirements within 30 days after publication of this notice. A determination by the Government to have competitive procurement based upon responses to this notice is solely within the discretion of the Government. (0038)
