Q -- FULL SERVICE PRESCRIPTION OPHTHALMIC LABORATORY Sol. Request for Proposal No. 5367-97907 (P). The Washington State Department of Social and Health Services (DSHS), Medical Assistance Administration, plans to issue a Request for Proposal (RFP) to obtain a contractor to implement an eyeglass frames and lenses procurement. The contractor must be a full service prescription ophthalmic laboratory with the capability to grind and finish glass and plastic lenses. The contractor must be able to process and deliver all standard prescriptions to the dispensing provider within ten working days of the prescription receipt. The qualified contractor must have been in the business for a minimum of three years. The bidder must furnish copies of contracts with the manufacturers of the contracting frames within 30 days after the contract is signed, and is required to generate performance reports on a monthly basis. The proposed release date of the RFP is February 15, 1995. Bids are due by or before 5 p.m., Pacific Standard Time, March 30, 1995. Bidders are solely responsible for the costs of preparing bids and DSHS shall bear none of the costs associated with such proposals.
