Contract base period is one calendar year from date of award with two (2) one year options for renewal. The contractor is required to provide radiation therapy consultation and treatment. This contract will include all hospital services including the supervision of the contractor's employees, supplies, equipment and resources necessary to provide for contracted services. Physicians who provide radiation therapy consultation must be board certified in this specialty by an American Board of Medical Specialty and apply for clinical priviledges at the VA. Provider's facility must be located within a 10 mile radius of the VA which is located at 7305 Military Trail, Palm Beach Gardens, Florida. Providers must have computers which can access VA Decentralized Hospital Computer System via modem and directly enter consults, treatment plans and assessments into patient's record. SIC Code: 8069.
