Rios, (210)221-0584, Fax(210)221-0259 The U.S. Army Medical Command, Central Contracting Office, has a requirement for twenty (20) each Patient Service Centers, Ophthalmology, for the new Brooke Army Medical Center, as follows: Universal Ophthalmic Instruments, Inc., Universal Infinity Refraction Desk, Model No 700-001 or EQUAL, to include: corian with tilt chair, refracting arm, slit lamp arm, overhead lamp, 3-well charges, indirect hanger and room lit control console (no sink); Twenty (20) Universal Infinity overhead custom wall cabinets, Model No 700-002 or EQUAL; Twenty (20) Universal Infinity pad/medicine racks, Model No 700-003 or EQUAL; Twenty (20) Universal Infinity RX viewer top inlayed, Model No 700-004 or EQUAL, and Twenty Universal Infinity keratometer arms, Model No 700-12164 or EQUAL. Hospital Grade Electrical Plug is required. Shipment FOB Destination. Turnkey Installation by contractor is required; On-site survey 30 days after contract award is required. On-site training is required for 20 operators/users and 5 Medical Equipment Repairers. This office plans to award a Firm Fixed Price Contract with an estimated delivery date to begin on 1 Jul 95.
