Q -- OUTPATIENT OXYGEN SERVICE SOL 569-8-95 DUE 041395 POC Contact Point, Marjean Litzenberg, (219)460-1411 Furnish, install and service all equipment, supplies and labor for outpatient oxygen program for veterans located in Indiana, and portions of Ohio and Michigan. Equipment and supplies to include a concentrator and related supplies and E and/or H oxygen cylinders. Contractor must be JCAHO accredited during entire contract period. It is estimated that 115 patients will be using this contract per month. These services will be provided to veterans being served by the VA Medical Centers in Fort Wayne and Marion, Indiana. Solicitation will be issued March 13, 1995. All requests for copies of the solicitation must be in writing. Estimated yearly cost of this contract is between $100,000 and $250,000.
