412/476-7396. The scope of work for the Bettis-Pittsburgh Site 1995
Maintenance Paving contract includes the resurfacing of approximately
7,000 square yards of existing pavement, the replacement of
approximately 2,200 square yards of existing pavement, the overlay of
approximately 500 square yards of existing pavement, the installation
of approximately 900 square yards of new pavement, the removal of 400
linear feet of existing curb, the replacement of approximately 3,400
linear feet of existing curb, the grading of two areas using soil
and/or aggregate, the line striping of three areas, and 90 tons of
pothole patching. Resurfacing efforts include the removal of 3-inches
of existing pavement and installation of 3-inches of new pavement.
Replacement efforts including approximately 100 square yards of new
pavement installation require the removal of 13-inches of existing
material, installation of 8-inchesof subbase and installation of
5-inches of new pavement. Overlaying efforts consist of the
installation of 1-1/2 inches or more new pavement atop existing
pavement. The remaining 800 square yards of pavement installation
consists of the installation of 3-inches of new pavement atop existing
grade. 1. All prospective bidder personnel must be U.S. citizens and
complete appropriate access paperwork (Questionnaire for Uncleared
Access-QUA) to attend the on-site pre-bid tour. Attendance at this
meeting is essential for proper understanding of unique ingress/egress
procedure and complex site conditions. Also, all vendor personnel
accessing the Westinghouse-Bettis site to perform work must be U.S.
citizens and any uncleared vendor personnel (i.e., currently not
possessing active Department of Energy security clearances) accessing
inside the Westinghouse-Bettis perimeter fence will be required to
complete QUAs. In this regard, in order for your company to receive a
copy of the RFQ package, completed QUAs for appropriate personnel must
be submitted to Westinghouse within three (3) calendar days from the
date of your request for a copy of the RFQ package. Westinghouse shall
process access requests at no cost to the contractor; however, the
effort to complete and submit QUAs is solely at the contractor's
expense. 2. Prospective bidders must have bonding capability from
sureties listed in the U.S. Treasury Circular 570 relative to contract
performance or be able to provide irrevocable letters of credit from
financial institutions that are Federally insured and have investment
grade or higher rating from a recognized commercial rating service. 3.
This is a small business set-aside action and all prospective bidders
must certify they qualify as a small business. 4. Prospective bidders
must agree to submit a current Foreign Ownership, Control or Influence
Certification and all necessary supporting business and financial
documentation, if requested by Westinghouse. Request for the RFQ along
with the necessary documentation clearly showing your firm meets the
above minimum prerequisites, must be in writing (telephone requests
will not be honored), and your letter must be postmarked no later than
