Contract base period is one calendar year from date of award with two (2) one year options for renewal. The contractor is required to provide consultation, treatment, and surgery. This contact will include all hosptial services inclusive of all fees including professional fees, the use of cardiac catherization lab and/or operating room along with supplies, recovery room services, cardiovascular perfusion, implantable hardware, central services, per diem room rates for the length of stay specified by the contractor, pharmacy, clinical laboratory, blood bank, respiratory therapy, ECG/Cardiographics, radiology, surgical pathology, echocardiography and graphics. Physicians who provide services must be board certified in this specialty area by an American Board of Medical Specialty and must apply for clinical privileges at the VA. Provider's facility must be located within 20 miles radius of the VA which is located at 7305 N. Military Trail, Palm Beach Gardens, FL. Providers must have computers which can access VA Decentralized Hospital Computer System via modem and directly enter consults, treatment plans and assessments into patients record. SIC Code:8062.
