Contractor will furnish monthly rental service of Home Oxygen Systems, which includes all labor, equipment and supplies to VA Beneficiaries within the Jurisdiction of the VA Medical Center, 2201 North Broadwell Avenue, Grand Island, Nebraska 68803; Bi-Monthly visits to each of the beneficiaries in accordance with all the terms, conditions, and provisions for the period of one year with two one-year renewable options. This service will begin with ONLY patients that are located 150 miles or more from the VA Medical Center, Grand Island, Nebraska. Patients less than 150 miles may be added to the contract at the convenience of the Government. The contractor must be available to service any veteran on an emergency basis at the request of the VA, including those normally serviced by the VA and are less than 150 miles from the VA Medical Center, Grand Island, Nebraska. This service includes but limited to concentrators, ``E'' cylinders, cannulas, oxygen carts, wheelchair carrier, oxygen regulators, Nasal CPAP and BIPAP, back up system for household failure and all other supplies required. Award will be made only to JACHO accredited facility. This is a 100% Set-Aside for small business concerns. Bid documents will be available approximately March 10, 1995.
