Simcoe Job Corps Center, White Swan WA. Services are to be performed at the Center. Services to include, but not be limited to: initial routine physical exam of corpsmembers, medical history, diagnosis and treatment of illness and/or injury, immunizations, X-rays, referrals to specialists, on-call services, 24-hour emergency coverage, access to inpatient care. Physicians performing services must be licensed in Washington State. The contractor will be required to perform a minimum of 30% of the services. All sources responding to this synopsis shall submit information relating to business status, including identification as to whether small, large, minority-owned, and/or women-owned.
