Work to be accomplished includes furnishing all labor, materials, equipment, and transportation necessary to manufacture and install on site, a 296 square meter modular Field Health Facility adjacent to the Eagle Butte Hospial at Eagle Butte, South Dakota on the Cheyenne River Indian Reservation. The faility will support the Optometery, Mental Health Education, Public Health Nursing, and Environmental Health programs. Site work shall be limited to the delivery and set-up of the modular Field Health building, installation of prefabricated steps and a single ramp, hook up to existing utilities and demolition or removal of the existing modular facility. Program of Requirements (POR) data will be presented in Metric within the Solicitation Package. The successful contractor will be required to submit detailed shop drawings detailing the proposed modular unit. These submittal drawings must confirm complete compliance with the metric POR prior to Notice to Proceed. Contract completion time is 180 calendar days after receipt of the Notice to Proceed. The proposed contract listed herein is being reissued as an unrestricted procurement. All responsible sources may submit a bid which will be considered by the agency. For work performed under this oontract, contractors will be required to give preference to Indian-owned firms for subcontract and supplier awards, and to Indians for employment opportunities.
