99 -- ISRAEL: DUE 050595 POC Tel 972-3-530-3030, FAX: 972-3-530-3568 901850 opthalmic instruments and appliances the chain Sheba medical Center invites suppliers to submit a quotation for an opthalmic laser machine tender no: -the machine must be mobile and it shall be possible toattach to it slit-lamps from the eye clinic's examining rooms and the operating rooms; -the machine must be operated with a cooling crystal to eliminate the need for a bulky, static cooling system, in order to provide for easy handling of the machine when preparing it for immediate operation in all areas of the eye department; -the machine must have a maximum capacity of 4.5 watt in the visible wave lengths; - the machine must be supplied with a single-phase 220V current at 50HZ; -the manufacturer must have a certified local agent knowledgeable in the field of lasers and capable of providing expertise, service and spareparts; -the manufacturer, through his agent, shall provide assurances for prompt service as follows a when receiving a call for service before 11:00 A.M., the repair shall be made the same day; B when receiving a call for service after 11:00 A.M., the repair shall be made the following day at 8 00 A.M; -the manufacturer, through his agent, shall provide assurances to supply the hospital with a replacement machine in case the repair takes more than 12 hours from the time of the call for repair; -the manufacturer, through his agent, shall provide assurances to keep a stock of spareparts for the repair of the machine within the specified timeframe; -the manufacturer shall specify the service required for the proper maintenance of the machine, the intervals at which this service is to be performed and the estimated costs involved in this service over a period of seven years (including the lifespan for the cooling crystal and its cost); - the machine must have an inclusive warranty period (labor and parts) for a period of 36 months. This warranty will become effective after a six month- running-in period for the machine in the hospital if in the course of the running-in period a problem arises with the machine which causes a complete interruption of the functioning of the machine, the running-in period will be extended by the number of days the machine is out-of-operation; -the manufacturer shall provide assurances regarding the availability of spareparts for a period of at least seven years from the date the warranty expires; -the machine must be supplied with all accessories.
