located at Alameda, CA, Seattle, WA and Kodiak, AK to beneficiaries
referred for treatment by a Coast Guard physicians. This will be a
non-personal services contract and medical liability insurance coverage
is required. This contract is subject to the Service Contract Act. In
Alameda and Seattle, the Contractor will work weekdays during hours
established by the Clinic Supervisor, normally 2 days per week. In
Kodiak, the Contractor will work weekdays during hours established by
the Clinic Supervisor, normally 1 day per week. Period of performance
will be 01 Oct 1995 through 30 Sep 1996 with three option years. The
RFP will be issued on or about 22 May 1995. This procurement is a 100
per cent Small Business Set-Aside. The SIC code is 8042 and the small
business size standard is $5 million average annual receipts for the
preceding three fiscal years. All responsible sources may submit a bid
which shall be considered by the agency. When calling, be prepared to
state the solicitation number. SPECIAL NOTE: For Minority, Women and
Disadvantaged Business Enterprises: The Department of Transportation
(DOT), Office of Small and Disadvantaged Business Utilization (OSDBU),
has programs to assist minority, women-owned and disadvantaged
business enterprises to acquire short-term working capital and bonding
assistance for transportation-related contracts. This is applicable to
any eligible prime or subcontract at any tier. The DOT Bonding
Assistance Program enables firms to apply for bid, performance and
payment bonds up to $1.0 million per contract. The DOT provides an 80%
guaranty on the bond amount to a surety against losses. Loans are also
available under the DOT Short Term Lending Program (STLP) at prime
interest rates to provide accounts receivable financing. The maximum
