Develop laboratory descriptive and test result collection instrument to package and mail to participating laboratories and provide CDC with SAS (Statistical Analysis Systems, Inc.) datasets. The period of performance is 24 months. Request for solicitation must be in writing. No telephone requests will be honored. The scheduled issue date is approximately May 30, 1995.
