Contractor to furnish all necessary resources, personnel, drugs, supplies, equipment and facilities for comprehensive home health care services to be provided to eligible Indian beneficiaries referred by the Indian Health Service (IHS), Uintah & Ouray Service Unit, FT Duchesne Indian Health Center, Ft Duchesne, Utah. In accordance with the IHS payment policy, contract awards will be made to those facilities which agree to accept as payment in full, reimbursement at rates no higher than the prevailing Medicare allowable rates (including deductibles and co-payment). Multiple contract awards will be made for a base year, with 4-one year options. Closing date for receipt of proposals is June 16, 1995 at 4:30 pm, local time.
