The Oklahoma City Area Indian Health Service desires to contract for a Basic Community Health Representative (CHR) Training course. The contractor is to provide two (2) consecutive, three week training courses. The contractor shall provide all lodging, meals, equipment, facilities, books, transportation arrangements and the cost to and from each students point of origin and some instructors for a minimum of eighty (80) participants.
