SE RVICES OF ON LOK, INC. SOL HCFA-95-062/PK POC Paula Kolick, Contract
Specialist, 410-966-5165. In accordance with FAR 6.302-1, The Health
Care Financing Administration's Office of Research and Demonstrations
intends to negotiate on a non-competitive basis with On Lok, Inc. to
continue access to a data system that On Lok has developed and also
owns, The Program of All-inclusive Care for the Elderly (PACE). The
PACE program replicates a model of managed care service delivery for
very frail community dwelling elderly, most of whom are dually eligible
for Medicare and Medicaid coverage and all of whom are assessed as
being eligible for nursing home placement according to the standards
established by the participating States. On Lok's sofware program,
called DataPACE, mainatains a database on PACE enrollees including
demographic and enrollment information, health and functional status,
and service utilizaton. For the PACE demonstration project, On Lok has
established a minimum data set and implemented data collection
procedures at the PACE sites. This database includes the variables and
program information originally designed to be used by evaluators. This
data has been purchased from On Lok for the past 3 years. Proposed
period of performance is 36 months from date of contract award.
