70 -- RENEWAL OF SITE LICENSE AND PURCHASE OF SOFTWARE/CONQUEST DATA SYSTEM POC Susan P. Moran tel: 301/443-6550/Jacqueline R. Jones CNT 213-95-0023 The DHHS/PHS/Division of Acquisition Management has a requirement for Strategic Mapping's CONQUEST Data System site license, support and maintenance and the purchase of software for the CONQUEST Data System. The Government only knows of one source, Strategic Mapping, Inc., 1001 G. Street, NW, Suite 350E, Washington, DC 2001, that has the capability of providing the following: software site license for CONQUEST and associated computer software. Specific products to be acquired are: CQ-DOS; CQ-DOS PHYSICIAN LOCATION - NATIONAL; CQ-DOS MDC/DRG FORECASTS - NATIONAL; and CQ-DOS HOSPITALIZATION UTILIZATION - NATIONAL. The Government may elect to accept, at no additional cost, later versions of the software, with software support to be provided for the remaining period of the requirement. The Contractor shall provide installation support for all software furnished. Maintenance shall include installation support and the Contractor, or its designee, shall include technical support for the implementation of CONQUEST on at least five free-standing personal computer. Hotline technical support service shall be provided on an on-call and as-needed basis. The anticipated period of performance is twelve (12) months from the effective date of the contract. This acquisition has been certified, justified, and approved in accordance with FAR 6.303. NO SOLICITATION IS AVAILABLE. THEREFORE REQUESTS FOR SUCH A DOCUMENT WILL BE CONSIDERED INVALID RESPONSES. Concerns having the ability to provide the software stated above must submit written notification that demonstrates their ability to satisfy the requirements. Requests shall be in writing to the attention of Susan P. Moran referencing Solicitation Number 213-95-0023. See Numbered Note 22. If no affirmative responses are received within 45 days from the date of publication of this synopsis, the Contracting Officer will proceed with this acquisition to Strategic Mapping, Inc.
