purpose of this procurement is to support the already established
community outreach models for the entire U.S./Mexico border area and
for improving access to health care delivery to the border population.
This will assist Federal, State border health offices, office of rural
health, and organizations to identify border health priorities for the
purpose of the planning process in their communities. Four mini-forums
will be conducted to assure that the outcome of health priorities will
be identified in each state. This opportunity provides the
introduction of a dynamic planning process for evaluating, establishing
priorities, agreeing on strategies and identifying the outreach model
for that community. To accomplish this process, each state where the
mini-forums will be held will include a collaborative project of health
care assessment and identify an approach for resolving the health issue
identified through the framework of a community outreach model. For the
purposes of this procurement and guidance, ''Community Outreach'' is
defined as follows: (1) a concept of collaboration, partnership, or
consortium; (2) a partnership of the area and community institutions
who are willing to work together to increase access to health care; (3)
a consortium of at least three local community institutions
collaborating on outreach projects; (4) community-based activities
which range from migrant health, rural health clinics, fire
departments, school districts, and university health centers. The main
goal of community outreach is to increase the accessibility,
acceptability, and appropriateness of available health services to the
U.S./Mexico border population. All responsible sources may submit a
proposal which shall be considered by the agency. Solicitation number
