F -- SUBSURFACE INVESTIGATION (SI) OF AN UNDERGROUND STORAGE TANK SITE, UTAH AIR NATIONAL GUARD BASE, SALT LAKE CITY, UT SOL DAHA42-95-B-0005 due 072795. The contractor shall provide the necessary personnel, equipment, and materials required to conduct a complete subsurface investigation (SI) to determine the vertical and horizontal extent of contamination of an underground storage tank site that has had a previous study together with remediation. The contractor must possess all necessary State of Utah certificates. The State of Utah has requested an additional investigation to assure that all contamination has been removed. The Contractor will (a) review site history and previous SI documents; (b) establish a schedule in accordance with the Statement of Work; (c) perform field investigation; (d) develop a draft SI report for review and comment; (e) develop a draft final SI report; (f) develop the final SI report for submission to the State of Utah UST section; (g) conduct monitoring IAW State of Utah requirements; (h) prepare quarterly monitoring reports for submission to the State; and (i) provide necessary documentation for site closure. This project is being considered for 100% set-aside for Small Disadvantaged Business (SDB) concerns. Interested SDB concerns should notify this office in writing, of their intention to bid on the project as a prime contractor as early as possible, but no later than 10 days from publication of this notice. The written response shall include a positive statement of eligibility as a small socially and economically disadvantaged business concern, the IFB Number, the firms complete name, address, and telephone number including area code, certified bonding limitation, capacity and current availability, dollar limit for bid guarantee/performance guarantee capacity, financial statement, provide a minimum of three references, to include title of project, dollar value, point of contact, and telephone number for the same or similar type work that is currently in progress, or that has been completed. If adequate interest and/or above information is not received from qualified SDB concerns by above date, this solicitation will be issued on an unrestricted basis without further notice. Therefore, replies to this office are requested from all interested business concerns as well as from small disadvantaged business concerns. Standard Industrial Classification (SIC) Code 8744 applies. The size standard for this SIC Code is 500 employees.
