The Federal Correctional Institution (FCI), Manchester, KY, intends to award a non-personal service contract for providing off-site Optometry services to the inmate population at the Camp for fiscal year 1996. Services will be provided at the Optometrist's office, on a scheduled appointment basis, the Optometric's will normally see approximately 10 patients each month, Monday through Friday, excluding holidays. The Contractor must be a graduate of an accredited School of Optometry program and possess current State of Kentucky licensure to practice Optometry. The Contractor's Office must be located within 30 miles of the institution. The contractor will provide, but is not necessarily limited to the following: Refractions, tonemetry, eye examination for abnormalities, referrals to Ophthalmologist when appropriate, and prescribing corrective lenses, eye medications as by law. The Federal Correctional Institution, Manchester, KY intends to award a base year contract in FY 1996 with options to renew the contract for four (4) consecutive years.
