Readjustment counseling will be provided in the form of individual, group and/or couple or family counseling. Services will be performed by a Psychiatrist, Psychologist, Licensed Social Worker, or other qualified personnel. Provider will be required to be physically located in Nevada County, California. SIC Code 8049 applies. Contract period is for a base year period beginning approximately October 1, 1995 through September 30, 1996 with two one year option periods.
