The Oklahoma City Area Indian Health Service is seeking sources for developing a Model Diabetes Program in order to evaluate the care and track outcomes of patients as necessary to establish a diabetes registry that encompasses information on all the diabetic patients within the Lawton Services Unit, including those patients being found with screening in the Strong Heart Study. This registry should allow providers to judge both the incidence and prevalence of diabetes and the occurrence of selected diabetic complications.
