Otolaryngology (ENT), shall provide professional services and expertise in the field to include medical and surgical treatment of the head, neck, ears, nose and throat. Conduct evaluations and referrals and make treatment recommendations to Health Services Staff. Minor surgical procedures may also be preformed. Services to be provided at the United States Penitentiary, Florence, Colorado. The contractor shall have the Education, Experience and Expertise which is consistent with the position of a otolaryngology (ENT). Contractor shall, if required be certified by the State Colorado. Request for the proposals must be in writing to include the solicitation number and name of requestor with address and phone number. Solicitation package will be issued on or about August 25, 1995.
