Scope of work includes, but is not limited to, renovate the patient rooms and toilet rooms to conform with directives addressing patient privacy in the 120 bed Nursing Home Care Unit (NHCU) at the VA Medical Center, Ann Arbor, Michigan. Work consists of the installation of semi-private baths with showers where rquired, enlargement of toilet rooms, movement of walls and finishes to result in room layouts which conform with new patient privacy requirements. Design shall be all inclusive to produce complete contract documents, submittal review, and construciton period services. Construction cost range is between $500,000 and $1,000,000. Qualifications submitted by each firm will be reviewed and evaluated based on the following criteria: (1) Team proposed for this project; (2) Proposed Management Plan; (3) Previous experience of team proposed for this project; (4) Location and facilities working offices; (5) Present proposed design approach for this project; (6) Cost control; (7) Estimating effectiveness; (8) Schedule control; (9) Describe your experience and capabilities; and (10) Miscellaneous: awards, circumstances and outcome of any litigations involved with in the last five years, and type and amount of liability insurance. Interested A/E firms located within a 60 mile radius of the Medical Center are to submit SF 254, A/E and Related Services Questionnaire, and SF 255, A/E and Related Services Questionnaire for Specific Project, to this office by close of business September 5, 1995.
