R -- DEVELOPMENT OF LOW LITERACY MODULES ON DIABETES SELF-MANAGEMENT TOPICS AND LOW LITERACY MODULES ON DIABETES AND PREGNANCY Sol REF NO. HQW074-0105/JA. The DHHS, Indian Health Service, Headquarters West, Diabetes Program, Albuquerque, New Mexico intends to enter into a sole source contract using the simplified acquisition procedures (FASA) to: (1) develop a series of low literacy modules on diabetes self-management topics; and (2) develop a series of low literacy modules on diabetes and pregnancy topics that will become the Indian Health Service (IHS) self-management series diabetes curriculum and Pregnancy Series Diabetes curriculum respectively pursuant to 41 USC 253 (c)(5) as implemented by FAR 6.302-5, Authorized by Statute, Buy Indian Set-Aside, 25 USC 47, and 41 USC 253 (c)(1), FAR 6.302-1, Only one responsible source and no other supplies or services will satisfy the Agency requirements. PROJECT 1 - Development of a series of low literacy modules on diabetes self-management topics that will become the IHS Self-Management Series Diabetes Curriculum. The curriculum will be used by IHS/Tribal health providers to teach American Indian/Alaska Native clients who have diabetes information and skills needed for self-management of their diabetes. The Diabetes Self-Management Curriculum will be the third in a series of curriculums on diabetes education developed by IHS and will follow the basic format of the first two (Basic Series Diabetes Curriculum and Complication Series Diabetes Curriculum). Each module will include a lesson plan to be used by the provider and patient education materials for client use. One module will be developed for each of the following topics: 1) high Blood Sugar (hyperglycemia); 2) Low Blood Sugar (hypoglycemia); 3) Diabetes Pills; 4) Giving One Kind of Insulin; 5) Giving Mixed Dose of Insulin, etc. PHASE 1 - RESEARCH: Contractor will review existing material already developed for the Self- Management Curriculum which includes objectives, some lesson plans in draft form, and some patient education materials and research current diabetes patient education curriculums and client print materials available from other diabetes patient education programs, government agencies (i.e. National Diabetes Information Clearinghouse) and diabetes, nutrition and fitness/exercise organizations/agencies (i.e. American Diabetes Association, International Diabetes Center, American Association of Diabetes Educators). PHASE 2: DESIGN: Contractor will solicit input from the IHS Diabetes Program Ad Hoc Executive Education (Nutrition Consultant Headquarters West (HQW) Diabetes Program, Nurse Consultant HQW Diabetes Program, Two Registered Dieticians (RD) IHS/Tribal Model Diabetes Program, Two Registered Nurses (RN) IHS/Tribal Model Diabetes Program, two Fitness Educators IHS/Tribal Model Diabetes Program, 1 RD generalist (IHS/Service Unit/Tribal clinic), and 1 RN or generalist Community Health Representative (CHR) (IHS Service Unt/tribal clinic) through conference call meetings to: 1) review diabetes self-management lesson topics; 2) review existing lesson plan objective; 3) develop lesson plans for each topic; 4) develop field test procedure and documents; and 5) review and revise the 19 draft lessons. PHASE 3 - FIELD TEST: Contractor will coordinate field testing of 19 draft lessons with health providers serving American Indians and Alaskan Natives with diabetes. IHS Diabetes Program HQW will assist contractor in identifying pool of health providers for field testing of draft lesson plans. IHS/tribal health staff will provide review comments to contractors. PHASE 4 - COMPLETION: Contractor with input from Ad Hoc Committee will review results of field test and incorporate changes suggested in field tests into final design. Camera-ready copy of all 19 lessons, client booklets and supporting computer disks shall be delivered to IHS Diabetes Program, HQW. PROJECT 2: Development of a series of low-literacy modules on diabetes and pregnancy topics that will become the IHS Pregnancy Series Diabetes Curriculum. This curriculum will include modules that address for major topics in pregnancy: Pre-pregnancy Counseling, Pre-existing Diabetes with Pregnancy, Gestational Diabetes, and Post-Partum. This curriculum will be used by IHS/Tribal health providers to teach American Indian/Alaska Native clients who have diabetes and are in their childbearing years and those clients who are pregnant and subsequently develop gestational diabetes. The curriculum will teach information and self-management of their pre-pregnancy condition, diabetes during pregnancy and post-Partum including the health of the infant. The Pregnancy Series Diabetes Curriculum will be the fourth in a series of curriculums on diabetes education developed by IHS and will follow the basic format of the first two (Basic Series Diabetes Curriculum and Complications Series Diabetes Curriculum). Each module will include a lesson plan to be used by the provider and patient education materials for client use. One module will be developed for each of the following topics: Section I: Pre-Pregnancy Counseling, Section II: Pre-existing Diabetes with Pregnancy, Section III: Gestational Diabetes Mellitus (GDM), Section IV: Post-Partum. PHASE I - RESEARCH: Contractor will review existing material already developed for the Pregnancy Series Diabetes Curriculum which includes some objectives, some lesson plans in draft form and some print patient education materials. The Contractor will research current diabetes and pregnancy patient education curriculums and client print materials available from other diabetes education programs, government agencies and diabetes/nutrition organizations/agencies. PHASE 2 - DESIGN: Contractor will solicit input from the IHS Diabetes Program Ad Hoc Executive Education Committee through conference call meetings to: 1) review need for all lesson plan topics and curriculum set up as proposed; 2) review existing lesson plan objectives; 3) develop lesson plans decided upon; 4) develop field test procedure and documents; and 5) review and revise the draft lessons. PHASE 3 - FIELD TEST: Contractor will coordinate field testing of all draft lesson plans with health providers serving American Indian/Alaska Native females who have diabetes, have diabetes and are pregnant, have gestational diabetes, or have had such a pregnancy. IHS HQW Diabetes Program staff will assist contractor in identifying a pool of health providers for field testing of draft lesson plans. IHS/tribal health staff will provide review comments to contractor. PHASE 4-COMPLETION; Contractor with input from the Ad Hoc Executive Education Committee, will review results of field testing and incorporate changes suggested in field testing into final design. Camera-ready copy of all lesson plans, client booklets and supporting compute disks shall be delivered to IHS HQW Diabetes Program. Period of performance is September 18, 1995 through September 20, 1996. No solicitation is available, therefore, requests for such documents will be considered invalid responses. Concerns having the ability to provide the services stated above must submit written notification that demonstrates their ability to satisfy the requirements.
