16 PLY, 25'' X 31'' inches. 16 tissue top sheet glued with single
bottom backing polyethylene sheets, 1000 Sheets per Bundle. QUANTITY -
96 BUNDLES. CONTRACTOR REQUIREMENTS--The Contractor shall provide all
labor, materials, equipment and supplies to provide the timely and
routine delivery of the cage/pan liners. The Contractor shall deliver
the cage/pan liners to the National Heart, Lung, and Blood Institute,
Animal Research Facility, 5 Research Court, First Floor Receiving Area,
Room 1A36, Rockville, Maryland 20852. The Contractor shall deliver the
cage/pan lines on an as needed basis for a twelve (12) month period to
begin upon award of the purchase order. It is anticipated that the
cage/pan liners will be delivered on a weekly basis. The Contractor
shall deliver the cage/pan liners within 48 hours of the Government's
telephone request. GOVERNMENT REQUIREMENTS--The Government shall
request delivery by telephone of a weekly basis of approximately one
(1) bundle. The Government shall furnish a forklift and pallet jack for
the removal of the cage/pan liners from the delivery truck. The
forklift and pallet jack will be operated by the National Institutes of
Health (NIH) animal husbandry contract personnel. REPORTS AND
DELIVERABLES--The Contractor shall submit a packing list with each
shipment indicating the number of bundles delivered. INSPECTION AND
ACCEPTANCE--All work shall be performed in a professional manner and
meet or exceed industry and/or government standards for performance.
The NIH animal husbandry contract personnel is the Government's
representative which will inspect and accept the quality of the
delivered product and the manner in which it was shipped. The liners
shall be inspected upon delivery by the NIH animal husbandry contract
personnel for cleanliness, soiling, quality and quantity. Any bundle
determined to be unacceptable will be rejected and must be replaced by
the Contractor at no additional cost to the Government. It is
anticipated that Request for Quottion (RFQ) number NHLBI-PS-SP-95-150,
will be available 15 days after the publication date of this synopsis.
Receipt of quotations will be due 10 days after the release of the RFQ.
The anticipated award date is 09/95. ''ALL RESPONSIBLE SOURCES may
SUBMIT A QUOTATION, WHICH IF TIMELY RECEIVED, SHALL BE CONSIDERED by
THE AGENCY.'' Requests for copies of the RFQ must be in writing and may
be requested by FAX to (301) 480-3345. Quotations must be submitted in
writing to the National Heart, Lung, and Blood, Institute, Contracts
Operations Branch, Procurement Section, Building RKL2, Room 6143, 6701
