Contact Kim Montgomery, Contract Specialist, 607/253-8830. Provide on
site services in the field of Physical Therapy at the Federal Medical
Center, Lexington, Kentucky. This is considered a contractual agreement
and not a personnel appointment. The estimated issue date of this
Request for Proposal is October 27, 1995. The contractor shall provide
on a non-discriminatory basis, services to provide physical therapy
evaluation, develop objective treatment plans, and provide
individualized treatment for patients referred to by the medical staff.
Provide rehabilitative evaluation and treatment for patients suffering
from a variety of orthopedic and neurologic problems, including but
not limited to: shoulder, hip, knee, back and neck pain, and
dysfunction, multiple sclerosis, AIDS, diabetes, arthritis, amputation,
spinal cord injuries, and sport injuries. Plan, implement, and
administer treatments utilizing various modalities including
therapeutic exercise, manual therapy, and physical agents (i.e. air,
water, electricity, sound and radiant energy). Develop plans for
individualized physical therapy regimens for pre and post operative
care and submit these plans to the medical staff. Design and teach
patients therapeutic exercises to prevent and resolve dysfunction in
the trunk and extremities and to restore maximum function. The
contractor will also utilize modalities and manual therapy as needed.
Contractor will advise the medical staff on the procurement and use of
special care devices and equipment for physically disabled patients.
Provide Quality Assurance and Quality Control input to the clinical
staff at the institution, as it relates to the Physical Therapy
Department. The contractor will see approximately 12 patients per
session. The contractor will arrange for an acceptable substitute
during periods of absence. Services will consist of the following
schedule: three sessions per week, a session being defined as six (6)
hours in length. The anticipated period of performance is from date of
award through September 30, 1996. All responsible offerors may submit
an offeror which will be considered. This requirement is set a side
for small business. Requests for information and/or the Request for
Proposal Package must contain the Request for Proposal number, business
