V -- REMOVAL OF SHARPS CONTAINERS SOL RFP 527-09-95 DUE 111995 POC Contracting Officer, Laralee Horowitz, (718)630-3568, Additional Contact, Fax No., (718)630-2969 Provide appropriate removal of Sharps Containers from the DVA Medical Center, 800 Poly Place, Brooklyn, N.Y. from date of award to September 30, 1996 with two option years for renewal. Vendor must provide at a minimum, one day per week full service, turn-key system including: a) Technicians must deliver containers and other required materials on the day of service (no prior storage permitted) b) Technician must collect and replace containers from all sites within the main building and all out buildings c) Technician must transport waste off the VAMC premises at completion of container exchange (no storage permitted, and subcontracting transportation of waste not permitted. The Standard Industrial Code is 4953. Requests for bid packages must be made in writing. Bid packages will be issued o/a 11/09/95. All responsible sources may submit a bid which shall be considered.
