786-9104. In accordance with FAR 6.302.1 the Health Care Financing
Administration's Office of Acquisition and Grants intends to negotiate
with the New England Medical Center (NEMC) to develop a survey to
collect data on case mix, patient satisfaction, functional status, and
self care of patients whose care gas been evaluated under the
Ambulatory Care Fee-for-Service Diabetes Project. In addition, NEMC
will provide consultation during survey implementation and assistance
in analyzing the data collected via survey. The Center for Clinical
Measurement and Improvement, Health Standards and Quality Bureau, is
conducting a national pilot project to evaluate and improve care
provided to patients with diabetes. Data will be obtained from
administrative and medical records. In addition, Health Care Financing
Administration plans to add a validated case mix method to the
project. This case mix will be derived via a patient questionnaire. The
questions will include components of patient satisfaction, functional
status, and self-care. These questions will be asked of as many as
7,500 beneficiaries in Alabama, Maryland, and Iowa. The project
timeline will require use of validated survey measures which have been
extensively tested in with patients with diabetes. The case mix must
be one which has shown to explain more than 35% of the variability in
outcomes among these patients. The entire survey package will have to
be available for implementation within 45 days of award of the
contract. The period of performance is one year from date of contract
