Contracting lab must have only one phone number for call ins and must be responsible for all phone line connections to their computer modem. Lab must supply mailers and packaging which includes approved infection control and biohazard packaging as VA form 90-2237-ADP, march 1985. SIC code is 8734 ($3.5 million) Contractor must provide daily pickup of both refrigerated and frozen specimens daily Monday through Sunday.
