Small Business Set Aside--This requirement is for the manufacture of
the formation light which is a non-flight critical component of the
Blade Tip on the CH-53 helicopter. Companies desiring to become
approved to manufacture the light will be required to submit a detailed
Source Approval Request together with the Request for Proposal to the
Naval Inventory Control Point - Philadelphia. The Source Approval
Request to be submitted shall include the following: A: Company
Brochure/Information containing a facilities listing, listing of
customer, number and type (i.e., Engineers, Machinists, etc.) of
personnel employed, listing and copies of contracts for relatable work.
B: Data such as work specification, contracts performed which
demonstrates experience in the manufacture and processes associated
with the manufacture of similar lights to include information
demonstrating the capability to develop process operation sheets and
inspection method sheets required to produce and verify a quality
light. C: A copy of the company's quality manual. This contract will
require the company to be compliant with MIL-I-45208A. D: Copy of the
company's performance rating from a prime contractor or military
service customer is requested. Questions relative to the submittal of
