The Albuquerque Area Indian Health Services to solicit proposals from prospective offerors for providing Optometric Health Care Services for the patients served by the Acoma-Canoncito-Laguna Indian Hospital, located on the Acoma Indian Reservation, New Mexico. The Government anticipates in awarding a single, requirements type contract for a base year with four one-year options for renewal. Contract period of performance will be for 12 months from date of award of a contract. Contractor will be reimbursed based on an hourly rate of actual services provided. The contract will be awarded as a personal services contract. Contractor must have the required Optometry License to practice from the State o f New Mexico or one of the 50 States within the United States, and be certified to use therapeutic ocular agents. Since the contractor will also be working with children and adolescent patients, pursuant to Public Law 101-630, Indian Child Protection and Family Violence Prevention Act, the contractor must submit to a complete background check through the National Agency Check and Investigation (NACI) process.
