Offerors will be expected to have the capability directly or through arrangements to provide physical assessments, medical treatment or therapy. Service providers submitting offers in response to this request for proposals must be physically located in LaSalle County, Illinois. Contract period shall begin approximately February 1, 1996 through January 31, 1997, with four option years through January 31, 2000. Bidding documents will be available after 12-4-95.
