This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposal are being requested and a written solicitation will not be issued. Solicitation 629-3-96 is issued as a request for proposal (RFP). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 34. The standard industrial classification (SIC) code for this acquisition is 3841. The small business size is 500 employees. Contractor shall provide one (1) each Alcon Series 20,000 Legacy Phacoemulsification Unit within 45 calendar days from date of award. Contractor shall provide manufacturers standard warranty to include labor and parts. System shall also include two (2) service and two (2) operator's manuals. The phacoemulsification unit shall contain the following salient characteristics: A peristaltic pump; a full function wireless remote control to allow for changes from the sterile field. The unit must have full programmability with a minimum of 245 surgeon memory and 96 distinct memory options; voice confirmation of mode selections is required; the extraction unit shall have tiltable, rotatable 256 minimum color monitor LDC touch screen. The contractor shall provide on site training for four (4) operator/user personnel and one Biomedical repair personnel. This training effort shall take approximately two (2) eight-hour work days and shall cover the operating and servicing data contained in the manuals. The training shall commence within 14 calendar days after delivery of the cataract extraction unit. Upon delivery the contractor shall notify the head nurse, operating room at (504) 589-5261 to solidify training plans. Commerce Business Daily Numbered Note 22 applies to this acquisition. It is the intent of this agency to solicit and negotiate with Alcon Surgical Inc., 6201 South Freeway, Fort Worth, Texas 76134. Delivery shall be FOB destination to the VA Medical Center, 1601 Perdido Street, Room BA116, New Orleans, Louisiana 70146. The Government contemplate award of a firm fixed price contract.
