C -- MODIFY AMBULATORY CARE SOL 626-18-96 DUE 012996 POC Contracting Officer, Susan Hilley, (615)327-5312 Design work to Modify Ambulatory Care for the Department of Veterans Affairs, 1310 24th Ave., S., Nashville, TN. This is a firm fixed price contract. This is not a request for proposals. Services contracted for will be preparation of contract drawings and specifications to Modify Ambulatory Care space of approximately 8,250 gsf, in order to correct configuration to provide adequate separation of ambulatory care services for patient privacy and confidentiality. Design work involves all major disciplines of arthitectural, structural, mechanical, electrical, plumbing and industrial hygiene. Estimated construction cost is $250,000 to $500,000. A firm fixed price contract will be issued for the site investigation and preliminary design work. VA will have an option to direct the A/E to complete the plans and specifications and perform the construction period services. Contractor will be required to have the following support functions; (1) spec writer/editor; (2) drafting/clerical; (3) design calculations; (4) working drawings; (5) as-built drawings; and (6) miscellaneous reports. Selection will be based on the following criteria which are listed in order of priority: (1) Team proposed: background of project manager, other key personnel and consultants; (2) Previous performance and provide references for the following work experience in order of priority: (a) VA Nashville (b) Other VA Medical Centers, (c) Hospital work and (d) Government work; (3) Proposed design approach for this project. Describe proposed design philosophy; describe innovative approaches in production and design; (4) Describe experience and capabilities in the following: value engineering, energy conservation, CAD drafting; (5) Cost control; (6) Schedule control; (7) Estimating effectiveness: submit a summary of the past 2 years which reflects your A/E estimate and Bid/Award price; (8) Proposed management plan; (9) Location and layout of facilities; (10) Described any awards for related work. Describe circumstances and outcome of any litigation that the A/E firm has been involved in the past 5 years. Indicate type and amount of liability insurance. Responses must be received by 1-29-96. Interested A/E firmsmust submit a completed SF 254 and SF 255 by 1-29-96, 4:30 p.m. (0352)
