NATIONAL TRANSPORTATION SAFETY BOARD FOR ACCIDENT/INCIDENT FILE

Transportation Safety Board intends to enter into an agreement with
General Microfilm to provide the following services. Store copies of
National Transportation Safety Board (NTSB) dockets in the form in
which they are received from NTSB, either on microfiche or
electronically as scanned images. Maintain and use an indexing system
to enable the identification of the appropriate microfiche or the
location of image records for an accident. Maintain and use a
computerized system for retrieving NTSB dockets that have been scanned
and stored in computer readable image format by NTSB personnel. Upon
receipt of a specific request, produce and delivery a copy (on paper or
in machine readable form) of any of the following: (1) the entire
contents (in monochrome) of a docket stored as either microfiche or
scanned image, (2) less than the entire contents (in monochrome) of a
docket that exceeds 100 pages in length, (3) a color Xerographic copy
of a scanned photograph, (4) an 8x10 inch color print using either
aperture card-mounted or loose negatives, (5) the most current type
(either preliminary or final) of accident/incident report form stored
as either microfiche or scanned image or produced as needed by
Government furnished equipment (GFE) software running on contractor's
computers and accessing the NTSB accident data base, (6) a probable
cause report stored as either microfiche or scanned image or produced
as needed by GFE software running on contractor's computers and
accessing the NTSB accident data base, (7) microfiche in the
contractor's possession, and (8) audio tapes or video tapes stored at
NTSB headquarters and available on loan to the contractor. When
requested, prepare copies for official certification by: (1)
hole-punching each page in the upper left corner, (2) securing all
pages to a heavy weight blue paper backing by GFE blue ribbon, (3)
completing a GFE form letter, and (4) delivering copies material with
completed form letter to a designated NTSB office where it will be
signed and mailed. Accept request via mail, package delivery service
(example, FedEx or UPS), messenger, telephone, fax, or E-Mail. Maintain
an automated customer tracking system for storing customers' names,
addresses, and other identifying information as needed by the
contractor along with a description of the order (to include as least
the information product(s) requested and the accident(s) for which
those products were ordered.) The customer/order record must be capable
of recording the fact that some products have been shipped while others
await processing. Accept payment by check and two major credit cards at
a minimum. Produce and ship copies within five working days after
receipt of order and payment (if required in advance), and availability
of the product requested. The contractor shall offer delivery via mail,
package delivery service, or (for small orders of hard copy products)
permit order pickup during the contractors' normal business hours.
Produce and ship copies within 24 hours when requested and when billing
and payment terms are agreeable to customer and contractor. Provide
weekly (by 12:00 noon on the second business day) to NTSB via
electronic file transfer, a file of names and addresses of customers
who have placed orders for products that are overdue from NTSB, the
NTSB identification number of the accident or incident, and the type(s)
of overdue product(s). Provide quarterly (by the fifth business day of
the calendar quarter) to NTSB via electronic file transfer, a file of
orders received or filled. Each record in the file shall consist of
the data for a single product type ordered (e.g., a preliminary report,
a complete docket, or a set of glossy photographic prints). The record
shall contain the product type, the NTSB identification number, the
date received, and (if the order has been filled) the date shipped. All
responsible sources may submit pricing information that will be
