Control and Prevention contemplates awarding a contract to implement
and maintain a biennial national school-based Youth Risk Behavior
Survey (YRBS) among 9th through 12th students during the springs of
1997, 1999, and 2001 and a national YRBS among selected populations of
high risk youth during the springs of 1998 and 2000. The YRBS System
monitors priority health risk behaviors that contribute to the leading
causes of mortality, morbidity, and social problems among youth and
adults in the United States. The six categories of health risk
behaviors are behaviors that contribute to injuries; tobacco use;
alcohol and drug use; sexual behaviors that contribute to unintended
pregnancies and sexually transmitted diseases; dietary behaviors; and
physical activity. The contract period of performance for the base
period is 16 months. In addition, there are four option periods for a
total performance period of 60 months. The base period and option
periods may overlap. Written requests for copies of the solicitation
will be honored if received within 15 days of the RFP issued date which
will be on or about March 14, 1996. Requests received after that date
will be filled on a first-come-first-served basis until the supply is
exhausted. The date specified for receipt of offers will be
approximately 30 days after the RFP issue date. Requests for the RFP
