The Plan will provide access to Primary Care at clinic sites located in rural areas for current VA patients residing in Minnesota. In addition to Primary Care services the following services in support of the Plan are required: referral management, case management, utilization management, claims processing, utilization reporting. It is expected that contract award will be made on or about June 1, 1997 with period of performance for the base year to begin on or about August 1, 1996 through July 31, 1997 with two options to renew for one year periods. By July 31, 1997, it is expected that 3000 Veterans will be enrolled in the Plan and by July 31, 1999, it is expected that 10,000 Veterans will be enrolled in the Plan. Offerors should be experienced in providing managed care and be able to provide access to several clinic sites in Minnesota.
