Provide physical therapy services at the Coast Guard clinic located at Petaluma, CA area to beneficiaries referred for treatment by Coast Guard physicians. This will be a non-personal services contract and medical liability insurance coverage is required. Contractor will work weekdays during hours established by the Clinic Supervisor, normally a minimum of 8 hours per week but not to exceed 16 hours per week, 248 hours maximum in the base year and 832 hours maximum in each of the option years. Period of performance for the base period will be 06 Jun 1996 through 30 Sep 1996 with two option years. The RFP will be issued on or about 08 Mar 1996. This procurement is a Small Business and Labor Surplus Are Set-Aside. The SIC code is 8049 and the small business size standard is $5.0 million average annual receipts for the preceding three fiscal years.
