The following information replaces and corrects that which was published in the two original synopsis. For each fiscal year, to include the base and four option years if exercised, the minimum amount of each delivery order will be $25,000.00, and the maximum amount of each order will not exceed $900,000.00. If this requirement is extended through a five year performance period, the minimum cumulative total of all orders issued will be $125,000.00, and the maximum cumulative total will not exceed $4,500,000.00. The statement, that this Agency intends to procure these services sole source from Kennedy Medical Offices which was cited in the synopsis published on 11/08/95, is incorrect.
