identification tapes, set of two (2) consisting of one (1) name tape
and one (1) U.S. Air Force tape. Type I (Lightweight) tape IAW
MIL-T-4566C, dated 01 August 1990. Twenty nine thousand, four hundred
(29,400) sets for the period 18 March through 01 June 96. Colors:
Tape--Camouflage Green 483; Embroidery--Flag Blue cable no. 67124. Bid
sample consisting of one each name and USAF tape will be required at
time of offer. Offerors will bid on the following items: CLIN 0001 --
Name tapes - 29,400 each and CLIN 0002 -- U.S. Air Force tapes - 29,400
each. Delivery required as follows: Training rosters will be provided
by the government with the Airman's last name. The training roster will
be faxed to or picked up by the contractor daily (M-F) as needed,
excluding holidays. The contractor shall deliver five (5) sets of the
name tapes/USAF tapes per trainee, no later than six (6) calender days
from receipt of the roster. FOB Lackland AFB, Texas, Building 5725,
Door 14. Contractor shall separate the name tapes by catagory with
corresponding invoices as follows: (1) REG=Active Duty, (2) GUR=Air
National Guard, and (3) RES=Air Force Reserve. Contractor will invoice
monthly for calls made during the previous month. The average training
roster will contain approximately fifty (50) to sixty (60) names per
flight. There is a possibility of a maximum of nine (9) flights in a
given day. Name tapes rejected by the government because of defects or
misspelling will be reaccomplished and delivered to the government
within two (2) normal duty days. Defective or misspelled name tapes
will not be returned to the contractor unless the contractor requests
they be returned and pays for the cost of returning them. Mistakes made
by the contractor will be corrected at no cost to the government.
Mistakes made by the government will be corrected at government cost.
Purchase exclusion IAW DFARS 208.7003-2(m). Tape and fabric must comply
with the requirements of DFARS 225.70002-1. This is a combined
synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6 as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; proposals are being requested and a written solicitation
will not be issued. This solicitation is issued as a request for
quotation. This procurement is reserved for small business
manufacturers or small business distributors of small business
manufacturers. This soliciation document and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
90-32. The provision at FAR 52.212-1, Instructions to Offerors --
Commercial, applies to this acquisition. The clause at 52.212-5,
Contract Terms and Conditions Required to Implement Statutes or
Executive Orders -- Commercial Items, applies to this solicitation.
Specifically, the following cited clauses are applicable to this
soliciation: FAR 52.222-26, Equal Opportunity; 52.222-35, Affirmative
Action for Special DIsabled and Vietnam Era Veterans; 52.222-36,
Affirmative Action for Handicapped Workers. The clause at DFARS
252.212-7800, Contract Terms and Conditions Required to Implement
Statutes or Executive Orders Applicable to Defense Acquisitions of
Commercial Items, applies to this solicitation. Specifically the
following cited clauses: 252.225-70001, Buy American Act and Balance of
Payment Program; 252.225-7012, Perference for Certain Domestic
Commodities. Offerors must include with their offer a completed copy of
the provisions at FAR 52.212-3, Offeror Representation and
Certification -- Commercial Items and DFARS 252.212-7000, Offerors
Representation and Certifications -- Commercial Items. The clause at
52.212-4 Contract Terms and Conditions -- Commercial Items, applies to
this solicitation. Copies of these provisions are available by
contacting the POC above. Award will be based on lowest price
technically acceptable offer. Quotes must be sent in writing or faxed
