Mack (706) 545-2219 (Site Code DABT10) Sources Are Sought To Provide The Following Services For Military Dependent Students Attending DoD Dependent Schools, Fort Benning, Georgia, As Ordered During The Period 01 Oct 96 Thru 30 Sep 97: 1. Non-Personal Services of Physical Therapist For Physically Impaired Students; 2. Non-Personal Services of Occupational Therapist For Students With Emotional, Intellectual, Physical, or Learning Disabilities. Both Services Shall Include Initial Screening, Comprehensive Assessment, and Annual Re-Evaluation of Each Individual to Determine Eligibility For Physical or Occupational Therapy. Services to be Performed Will Be Determined Based on the Number of Students Requiring Each Service. Anticipated Number of Students Requiring Each Service is from 10 to 15. The Anticipated Frequency of Each Service is from 8 to 24 Hours During 1 to 3 Days Per Week. Estimated Annual Requirement For Each Service is 600 Hours.
