Testing Services and Clinical Pathology Consultation Services for
twelve (12) USCG Coast Guard clinics located in Alameda, Petaluma, San
Pedro & Humboldt Bay, CA; Astoria & North Bend, OR; Seattle & Port
Angeles, WA; and Juneau, Kodiak, Ketchikan & Sitka, AK. These services
shall include pickup and transport of specimens and to it's
laboratory, pre-analytic processing, analysis and result
interpretation. This will be a non-personal services contract and
medical liability insurance coverage ($1,000,000.00) is required.
Period of performance for the base period will be 01 Oct 1996 through
30 Sep 1997 with four option years. The RFP will be issued on or about
15 May 1996. For the convenience of offerors, the proposal schedules
will be provided on both paper and 1.44 MB floppy computer discs
formatted as Lotus Version 2.1. Offerors may utilize this version or
convert spreadsheets up to either Lotus Version 5.0 for windows or
Excel, Version 5.0 for windows. There is a non-refundable cost of
$20.00 for each proposal package requested. Your request for package
must be accompanied by a company check made out to the US Coast Guard.
This procurement is open to all prospective offerors, both large and
small business. The SIC code is 8071 and the small business size
standard is $5.0 million average annual receipts for the preceding
three fiscal years. All responsible sources may submit a bid which
shall be considered by the agency. SPECIAL NOTE: For Minority, Women
and Disadvantaged Business Enterprises: The Department of
Transportation (DOT), Office of Small and Disadvantaged Business
Utilization (OSDBU), has programs to assist minority, women-owned and
disadvantaged business enterprises to acquire short-term working
capital and bonding assistance for transportation-related contracts.
This is applicable to any eligible prime or subcontract at any tier.
The DOT Bonding Assistance Program enables firms to apply for bid,
performance and payment bonds up to $1.0 million per contract. The DOT
provides an 80% guaranty on the bond amount to a surety against
losses. Loans are also available under the DOT Short Term Lending
Program (STLP) at prime interest rates to provide accounts receivable
financing. The maximum line of credit is $500,000.00. For further
