The Contractor shall furnish dental services for the students of the Blackwell Civilian Conservation Center. This includes all supervision, labor (including any dental assistant or hygienists), and incidentals. Including but not limited to routine dental examination, providing of a diagnosis and treatment plan for each student, providing preventative dental health education, providing 24 hour emergency coverage and advice to the Center Director. The services shall be provided at the Contractor's facilities, no dental facilities are available at the Center. The Contractor facility shall be within approximately 50 miles of the Blackwell Center. The Blackwell Center is located approximately 5 1/2 miles south of Laona, Wisconsin. The solicitation consists of a base year beginning on July 1, 1996 and four optional years each beginning on July 1, and ending on June 30, until the year 2001. (0065)
