Space Administration (NASA) Headquarters' Office of Safety and Mission
Assurance is currently seeking vendors who can provide information
about their off-the-shelf software with capabilities of collecting,
tracking and reporting on mishap, accident, and injury cases that
affect the safety of NASA civil servants, contractors, and visitors.
This software is intended to replace the current agency-wide Mishap
Reporting and Corrective Action System. The significant Functional
Requirements are as follows: (1) Provide for maintenance of mishap
information which include general information about personnel involved,
description, date, time, location, responsible organizations, equipment
damaged, category of incident; (2) Provide for immediate electronic
notification of all new incidents to each responsible office for
review, track each incident reviewed by the offices, and record how
each office has classified the incident; (3) Provide the capability of
assigning an incident to multiple categories (e.g. severity, loss
time, first aid, mission failure) and provide the capability of
customizing classifications to fit changing needs; (4) Provide an
updateable electronic distribution list to provide immediate
notification to the responsible officials based on the classification
of the incident; (5) Provide for maintenance and tracking of corrective
action plans including each activity, responsible person and due date;
(6) Provide the capability of tracking and reporting on incidents
involving civil servants, on-site contractors, off-site contractors,
and persons not affiliated with NASA as separate groups and
collectively; and (7) Provide for maintenance of information to support
the data requirements for worker's compensation claims and completion
of Department of Labor forms. The significant System Requirements are
as follows: (8) Provide for independent installation and operation at
remote sites and data integration at a central site; (9) Provide a
simple and reliable method for electronically transmitting and
integrating the data between sites; (10) Provide each site with the
ability to selectively choose the type of data to transmit and the
capability to sanitize the data to comply with Privacy Act requirements
prior to transmitting to the central site; (11) Provide all sites with
access to the sanitized data transmitted to the central site for
comparison, analysis and reporting; (12) Provide a Graphical User
Interface (GUI) to access all the workstation functions; (13) Provide
the capability of sharing data with other desktop applications and
client/server applications; (14) Operate on Microsoft Windows 3.11 and
Windows 95 operating systems; (15) Utilize a distributed client/server
architecture; (16) Utilize a Local Area Network (LAN) and a Wide Area
Network (WAN) using TCP/IP protocol; (17) Support up to 20 concurrent
users per-site and 100 concurrent users connected to the integrated
site; (18) Provide system administration features at each site which
provides multiple levels of system access and security (e.g. limit
access to the system, common data, site specific data, function
specific data, Privacy Act data); and (19) Provide a means for
migrating Clipper/dBASE III files from the existing legacy system into
the system's standard format. The significant Reporting Requirements
are as follows: (20) Provide the capability of producing applicable
OSHA forms and logs (daily log and 200 log); (21) Provide the
capability of producing applicable Department of Labor worker's
compensation forms (CA-1, CA-2, CA-2a, CA-3, etc.); (22) Provide the
capability of producing the NASA 1627 form (NASA Mishap Report) or
comparable equivalent; (23) Provide the capability for reports to be
viewed on screen, printed, and exported to other file formats; (24)
Provide built-in or seamlessly integrated ad-hoc querying and reporting
tools; and (25) Provide for creation and tailoring of standard reports
at each site, some of which include graphs and charts. All interested
