The Contractor shall furnish mental health services at the Pine Knot Civilian Conservation Center. This includes all supervision, labor (including any dental assistant or hygienists), transportation of Contractor's personnel and incidentals, other than those provided by the Government, necessary to successfully provide mental health services. The consultant must submit proof of certification, licensing or accreditation in the Commonwealth of Kentucky before award of the award. A copy of the Consultant's current license must be attached to the resulting contract. This solicitation consists of a base year beginning on July 1, 1996, and ending on June 30, 1997. Optional years may be executed from July 1, of each year until June 30, of the next year, until June 30, 2001. The project is located at the Pine Knot Civilian Conservation Center, U.S. Highway 27, Pine Knot, Kentucky. (0068)
