Chapman (910) 396-4362 X237 (Site Code DAKF40) The U.S. Army has a requirement to provide Physical Therapy services for students grades Pre-school through Grade 9 enrolled in the Fort Bragg Schools, Fort Bragg, NC and Pope Elementary School, Pope AFB, NC. These Physical Therapy services shall consist of a therapeutic approach involving screening, consultations, evaluations, and the constant process of re- evaluation using techniques, activities, and objectives to reflect the change of students needs during the physical therapy process. The performance period for this requirement is for a base period of 15 Aug 96 - 30 Sep 96 with two one one-year option periods. This is a full and open requirement. Large and Small Businesses will be considered for award. Requests for solicitation must state bidder's business size and that vendor is not currently on a consolidated list of debarred, suspended and ineligible contractors or otherwise ineligible to receive Government contracts. The Standard Industrial Classification code for this requirement is 8049. To be considered a small business under this classification, a firm, including its affiliates, must not have more than $5 million average annual revenues over the past three years.
