Contract period is for one base year plus four option years beginning 1 October 1996. Provide occupational job related medical services of the type commonly furnished DOD employees, to include employment related services and examinations at assigned geographical duty locations in the following states: Alabama, Arkansas, Connecticut, Delaware, Florida, Georgia, Indiana, Kentucky, Louisiana, Maine, Maryland, Massachusetts, Michigan, Mississippi, New Hampshire, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Rhode Island, South Carolina, Tennessee, Vermont, Virginia, and West Virginia. Contract performance includes operation of Health Clinic on site at DCMD East Boston, Monday through Friday 7:00 a.m. to 5:00 p.m. and necessary clinical support to service duty locations identified above. All medical personnel must meet state licensing requirements in the states where the services will be provided. Services rendered will be in accordance with Federal Occupational Medical Standards and OSHA, DOD 6055. manuals, etc. All responsible sources may submit a proposal which shall be considered by the DCMD East-Boston.
